Why is my platelet count so low?

Dear Dr. Chung,

Once diagnosed with Hep C I went through a series of blood test. It showed that my ATL, enzines, and viral loads were extremely high with a platelet count of 66. I had a trans jugular biopsy that should stage 3/stage 4. I am soon to start combo therapy. My doctor plans to keep me on therapy only until my platelets drop to 35-40 then he plans to stop therapy. My question to you is, why is my platelet count so low? - I haven't even started therapy yet!

Two possibilities. you may have advanced liver disease with portal hypertension - with congestion in the spleen causing reduction in your platelet counts. HCV itself may also be associated with platelet drops through mechanisms that aren't clear. in some cases we have actually seen platelets rise on therapy , so good luck.
