Do you have any information at all regarding treatment in transplant patients?
 
 
Dear Dr. Chung,
 
 
 
I have HCV diagnosed in 1993, received a transplant in 1994, The new liver was reinfected rapidly, and even though LFT's remained normal I developed cirrhosis again within 3 years. My kidneys were severely damaged and in 1999, I recieved a secon liver and a kidney from the same dondor. My viral type is #3. A 9 month course of combination treatment in 2000-2001 eliminated the virus but I relapsed in Oct. 2001. Have been on PegIntron and Ribavarin since then. My question is this - Do you have any information at all regarding treatment in transplant patients? MY viral count is currently undetectable, but if I relapse again, would you recommend a third round of treatment? Biopsies every 3 months have shown stage 2 inflammation and fibrosis, although there doesn't seem to be any progression of damage while I am on therapy. I would appreciate any info you might have in this area.

This may be one situation that if you relapse again that you consider chronic maintenance therapy to maintain graft function - we have done that in a number of our more severe HCV post transplant recurrences.
